What this policy covers

This policy explains what happens to information when you use this website, call us or write to us. It applies to Reign-Bow Brain Treatment Center, 55 W 22nd Street, Lombard, IL 60148.

It does not cover the health records of our patients. Those are described in the next section.

Your health information

If you are a patient, the health information in your records is protected by a federal law called HIPAA. How we may use and share it, and your rights over it, are set out in our Notice of Privacy Practices. You can read it on our HIPAA notice page, or ask for a paper copy at our office.

This website policy does not replace that notice. Where the two differ about patient health information, the Notice of Privacy Practices applies.

Information you give us

We receive the information you choose to send us:

• through the consultation and insurance forms: your name, phone number, email address, who the request is for, an age group, the service you are asking about, your insurance company and plan type, and the best time to call
• in an email you write to us
• in a phone call

When you press the button on one of our forms, your answers are placed in an email addressed to us and opened in your own email app. Nothing is sent until you press send, and the message travels through your email provider like any other email.

Please leave medical details, member ID numbers and dates of birth out of forms and emails. We collect those by phone. Email is not a fully secure way to send sensitive information.

Information collected automatically

Like almost every website, the company that hosts this site keeps standard server logs: the internet address of the device asking for a page, the browser type, the page requested and the time. We use these logs only to keep the site running and secure.

Cookies and analytics

This site does not use advertising cookies, and it does not currently use analytics cookies.

If that changes, we will ask for your agreement first and update this page.

Other services this site uses

• Typefaces. The fonts on this site load from Google Fonts. Google’s servers see the internet address of your device when they deliver them.
• Maps. The map on our Contact and Locations pages comes from Google Maps. It loads only if you choose to show it or have accepted cookies. Links to directions open Google Maps, which has its own privacy policy.
• Email. Messages you send us pass through your email provider and ours.

How we use information

We use what you send us to reply to you, schedule consultations, check insurance benefits when you ask us to, and improve this website. We also use it where the law requires us to.

We do not sell your information. We do not share it with anyone for advertising.

Who we share it with

• Companies that provide services to us, such as website hosting and email, which may handle information only on our instructions
• Your insurance company, when you ask us to verify your benefits
• Authorities, when the law requires it

How long we keep it

We keep requests sent through this website for as long as we need them to respond, and longer where the law requires. If you become a patient, the rules for medical records apply.

Security

We take reasonable steps to protect the information we hold. No website or email system can be guaranteed secure, which is why we ask you to keep medical details for the phone.

Children

We treat children, but this website is written for adults: parents, guardians and adult patients. We do not knowingly collect information directly from children under 13. If you believe a child has sent us information, contact us and we will delete it.

Your choices

You can decline cookies and still use every part of this site. You can ask us what information you have sent us through this website, and ask us to correct or delete it, by calling 630-448-2721 or writing to care@reignbowtreatmentcenter.com. Requests about patient records follow the Notice of Privacy Practices.

Changes to this policy

If we change this policy we will post the new version here and update the date at the top of the page.
