We are required by law to protect the privacy of your health information, to give you this notice of our legal duties and privacy practices, and to follow the notice currently in effect. The full notice is in the PDF above; this page summarizes it.
How we may use your information
We may use and share your health information to treat you, to bill your insurance, and to run the practice, for example to coordinate care with your other providers or to review the quality of our care. Some uses are allowed or required by law without your permission, such as public health reporting or responding to a court order.
When we need your written permission
Most uses of psychotherapy notes, any use for marketing, and any sale of health information need your written authorization. You can withdraw an authorization in writing at any time.

- Ask us to contact you in a particular way or place
- Get a list of certain disclosures we have made
- Get a paper copy of this notice
- Be told if your information is involved in a breach
